A new white paper was published to present the significant benefits of Samepage
Healthcare’s Collaborative Care Solution. It describes in detail the clinical outcomes and results of the collaborative care pilot project ran in Greenfield Health in Portland, Oregon.
Introduction:
Greenfield Health (Portland, Oregon) recently deployed Samepage Healthcare’s Collaborative Care Solution to treat 43 patients with psychiatric issues (depression, anxiety, ADHD, substance abuse) and co-morbid medical conditions (e.g. diabetes, hypertension). Over enrollment periods of 22 weeks, 88% of depressed patients achieved non-depressed status. Among patients with out-of-target glucose control, 88% of patients achieved target glucose control. There was an 82% reduction in enrolled patients who had blood pressure readings in the hypertensive range. The pilot also demonstrated that using Samepage’s solution, a full-time care manager will have the capacity to efficiently manage approximately 190 patients per year.
